Before Your Appointment
Each person must complete the online patient registration form before the appointment.
Cost: $30 per person. Cash or prepayment ONLY
Things To Remember
- Each person in your vehicle who will be receiving a COVID-19 Rapid Antigen Nasal Swab Test, must have already pre-registered.
- Bring your ID.
- If you prepaid, bring your receipt or bring $30 cash per person.
- Everyone in the vehicle ages 2+ must wear a mask.
- Do not bring any pets.
- For children, we recommend that an adult is in the back seat with the child if possible.
When You Arrive
When it’s time for your appointment, make sure you have your ID ready. Everyone in the car ages 2+ should be wearing a mask.
Do not get out of your car.
Our team will administer the COVID-19 Rapid Antigen Nasal Swab Test right through your open car window or door.
Once your vehicle is under the tent, make sure your car is in park and turn off the engine.
